Pull Down Menu Help

I need to create a pull down menu from a list of names from a database.  I also need an option within the menu to add a name if it is not listed in the database without refreshing the screen.  Any examples of how this is done?

Pull down menu example:

$query = "SELECT name FROM users WHERE id='$id'";
$result = mysql_query($query)or die(mysql_error());

if ($result){
    echo '<select name="name">';
    
    while ($row = mysql_fetch_assoc($result)){
        echo "<option value='{$row['name']}'>{$row['name']}</option>";

Insert new name example:

if (isset($_POST['submit'])){
    if (isset($_POST['name'])){ 
        $name = mysql_real_escape_string(trim($_POST['name']));
        
        $query = "INSERT INTO table (name) VALUES ('$name')";
        $result = mysql_query($query)or die(mysql_error());
        
        if ($result) echo "Name was added";
        else echo "Something went wrong with the query...";
        
    } else {
        echo "You left the name blank!";
    }
}
                    
?>

<form method='post'>
    Name: <input type="text" name="name"><br>
    <input type="submit" name="submit">
