i dont have a script at hand, but if you break it down into stages it doesnt seem so hard:

 

1. take all the details from the form

2. insert them into a database, with a field for a random number, and a field 'active' and set it too 0.

3. use the mail funtion to send an email and contain a link to like www.yourdomain.com/activate.php?randNumber=3453453533455

4. Write a script called activate.php where you will $_GET['randNumber'] and then update the database so that active= 1 where random number = $_GET['randomNumber']

 

that is a process I have used, but you should add some other security features such as validating input into database
