Checkboxes and updating sql data

Hello,

 

I am trying to do the following, in the most simple manner possible, since my php knowledge is quite limited.

 

I am trying to create a form with checkboxes. Once the checkboxes are clicked and submitted, it updates the table. Should the person return (the site has a login feature already), the form appears with their previous entries and allows them to add/remove entries before resubmitting it, which updates the db with the new info.

 

Does anyone know of anything "off the shelf" that might do the job?
