Uploading images and naming to directory

Hello coders = I am new here = this looks like a great forum!

 

I am searching the posts and have found a lot of stuff on uploading images.  Could someone who has been here a while point me to any threads that report on:

 

1) Uploading an image to a directory AND naming the image in a database table in one fell swoop.

2) How I might code deleting the image in the directory IF the record in the db is deleted.  Actually I want to set up 5 fields for 5 images per record - the unlink() function must purge ALL 5 fields and rename the default string to "no_pic.jpg" - or which ever funtion is called to purge.
