OK, I moved the connection script closer to the page and that generated the following erro

 

You could not be registered due to a system error. We apologize for any inconvenience.

Yes

 

I don't know why the test connections work but the form will not connect.

 

The bit

 

$database_select_1 = "partners"; //the name of the table being used

 

You wanted the table name not the database name? partners is the name of the table and the database is named db100162_HAreg

YES! ok, while I was waiting I went ahead and switched the table to the name of the database and now I have the error
