CSS is going to make the biggest impact on layout and theme issues, so I would start there if you haven't done that yet. The rest of templating is usually a case-by-case basis, so you have to have a starting 'look and feel' before we'd be of much help. If you're really at a stop point, I'd start googling some site templates and find a few that catch your eye, then modify accordingly. Templating is all about what YOU want, not what we would want for you. = )
