If they leave it empty then it returns with a error code (Ex: * First Name not entered, etc) Well even if I do put something in the field, it returns with the errors. What I mean by that is: $subfirst, $substore etc aren't defined so empty($subfirst) and empty($substore) are always true. 2. That php file that you posted isn't really relevant. Where's process.php?
