This is a good tutorial on how to set up PHP5 on W2K3,

You would need to NAT an external IP address to your server either via your router or a hardware firewall and set up additional security on your server just to lock it down.

You'll also likely want to consider a service such as dyndns since you don't have a static IP.

 

Also, might read the TOS/AUP of your ISP as a lot of ISP's do not allow you to run servers from your home connection...some of them even block port 80.
