PHP Captcha form and Frontpage

Hi

I would like to use this form http://www.thewebhelp.com/php/php_contact_form_with_image_validation/.

I already have a index.html and would like insert this form in a cell in a table using FrontPage, but I dont know how to do it.

I would like to have the Subject field filled in with xxxxxx.
