Help wanted here. phpBB variation. (basic question)

Hey newbie here, i just want to ask how to make an implementation of a posting like system like this:

 

Every time that a poster fills a form like this:

 

 

 

It automatically adds the post like this:

 

 

 

 

 

Then when is automatically added the post will look like this :

 

 

 

 

Its like a posting system of a forum, but slightly different. How can i do this ?

What is that technology? A variation in the code of phpBB ? Any good sources? thanks.

This is the basics.

You make a mysql table that contains fields that will store the information from each input field of that form. When the form is submitted, you use mysql_query to store the data from the dorm in that database. If you want a photo, it is a little more complicated but pretty easy still. You then call the information into the respective html page.
