When the user fills out the info and presses submit, I need the info to be inserted into a database. Here's the info: Database name: FP Table: Rate Fields: -Username - a name -Prof - a link -Age - a number -Date - a date (MM/YY) -Smart - another number When the user goes to the page and fills out the information asked, it should be inserted into the corresponding fields.
