By isolate i mean remove the parts that have nothing to do with the query. I don't have time to go through your entire script line by line, so here's what i think you need to do.

 

if (count($columns) != count($values))

If this doesn't help, try understanding what your own code does a little better. Gotta learn to crawl before you can walk.
