Need Help Getting A Template Contact Form To Run

I bought this HTML/Flash template with a "Contact" form layout and buttons (so people can submit questions to my Web site). It has a Contacts.php file and a Contacts_config.php file...but the form doesn't work. The code seems to have all the structure in place, but I'm missing a few pieces. Can anybody help get this form to work? Thanks!
