File that was changed to html index..htm

it was renamed to .htm which changed it to an html file. then i opened the file in dreamweaver and saved it to php because renaming it did not work when was uploaded it did not work correctly

someone converted this to html and now that i went back it does not work at all i am very new please help Index.php
