  1. The values of shell environment variables can be used in configuration file lines using the syntax ${ENVVAR}. If "ENVVAR" is the name of a valid environment variable, the value of that variable is substituted into that spot in the configuration file line, and processing continues as if that text were found directly in the configuration file. (If the ENVVAR variable is not found, the characters "${ENVVAR}" are left unchanged for use by later stages in the config file processing.)

  2. "last-of-type " should be applied on <p>.

    if the form submits and post data to a second page, you may want to put it at the top of both pages. also, make sure the code is after any session_start(); if you are using sessions.

  10. I think its the validation code that are not accepting special characters.FInd form code and add all those special characters in the code , (for your convenience, see where alphabets letters listed. ) 

  11. You question is not so clear.Anyway, seems its a simple inner join case.An SQL INNER JOIN return all rows from multiple tables where the join condition is met.

    Example:

    SELECT Orders.OrderID, Customers.CustomerName, Orders.OrderDate
    FROM Orders
    INNER JOIN Customers
    ON Orders.CustomerID=Customers.CustomerID;
    

    Order:Table 

    OrderID   CustomerID   OrderDate
