If I know the path of a file on the client's computer, how would I go about uploading this file to the server without using an upload form?

I want to create a site that manages large amounts of audio and video content. Is it possible to store such content in a database? Or is the best solution to just stick all the audio/video files in a folder on the server? I know that dealing with files is slow... so... how do sites like Launch Music, Google Video, and YouTube do it?

I noticed that when i visit facebook (or many other sites, for that matter), i get a link that terminates with .php?id=(number) example: http://ufl.facebook.com/profile.php?id=1234567 Why does the url terminate that way? What i'm trying to figure out is how exactly sites like facebook/myspace store the user information. Is it stored in a database and retrieved every time the user's information is requested? Is the .php?id part like a session id, or what?
