Let's say I have a URL like:

mysite.com/index.php?page=cat&name=friends&episode=5

 

Which of the above would you think is the most user friendly and best optimized for search engines. Feel free to state a different one as well.

They are all the same as far as SEO goes except for the inclusion of the word episode in some of them, so if you want that keyword, use a URL with that.
