$_SERVER['REMOTE_ADDR'] returning my server's IP instead of the visitor's

As the title states, I've noticed that occasionally my site shows the server's ip address from $_SERVER['REMOTE_ADDR'] instead of the visitor's and it's weird because it only happens about half the time.  Any idea what's going on?  I'm guessing it's my server's IP, i don't know what else it would be because it's a private ip range (192.168.151.33)

could it possibly be when you are making changes to the site via ftp?
