File Uploading Script Issue

I wrote a script that i use on my WAMP, and it works jus fine there, but I am having trouble getting it to work on the website server.  I have double and triple checked this code, and been working it for two days now!! Im out of ideas...  Any suggestions guys?

did it give any errors?

or is it

"simply not working"......

Make sure the directory is correct, and that it is writable on the server.
