They click submit and information is sent to "contact us" webpage opens in the browser and their name and stuff populates a regular contact us webpage made like this http://dreamweaverspot.com/adobe-dreamweaver-tutorial-contact-forms/ is what I am following. In the attachment have the user enter their name (John Doe) and click a done button and have John Doe appear on the Dreamweaver form. Then use something like Formmail.php contains an upload button. In the PDF there is a field with the file path so their name and the upload field would contain the file path. Then enter in any comments. Select the person it is suppose to go to.
