Image Upload Issue

I'm quite the newbie with PHP. I want to create a two page php site. The index.php will be the page where the user can upload the image. The image_list.php is where the user can see the uploaded images. The actual upload works fine. Getting the images to display on image_list.php is where im running into problems. Where would i insert the following:
