Disappearing html problem/php includes

Hello. The index page for my website contains both html makeup and a php include pointing towards a blogging script. When I check my code and visuals in DW MX everything looks peachy and clear. However, once I upload the index.php file to the server some of my html completely disappears.

When I view the page source in Firefox to review the code which should be in the rightmost table (which appears pitch black and empty) it is completely gone.

When I download the index page directly to my computer and recheck it in Dreamweaver the code is there. Same applies for the html editor built into my cPanel file manager.

Also, when I remove the php include and change the file name back to .html all of the code appears live (as seen here: [url=http://www.beattheindiedrum.com/index.htm]http://www.beattheindiedrum.com/index.htm[/url])

So, are there settings on my web server that I may be overlooking to remedy this or do I just have a bad case of phantom code? I am guessing the php include has something to do with this since removing it clears the problem but I really don't see how that's possible. There was a period in time when this all worked flawlessly and effortly but then all of a sudden there was a problem. Any help would be super-appreciated.

No, there isn't a server setting in charge of this, we need to see code.
