Form Processing script?

Hello,

 

I'm looking for a Form Processing script.

I used a perl script (soupermail) but now it's too old (not more safe), thus I search the same features in PHP.

I've found different open scripts on the Web, but none with all features, and I don't have the Programming Knowledge to merge them together!

Would you have an open script to refer to me?

Some features that I need:

- different field types including Text Boxes, Passwords, Selection Menus, Check Boxes, Radio Buttons, Text Areas and File Uploads,

- input validation (if required),

- page redirect (confirmation page) and sender notification (auto-response, confirmation email),

- plain text or HTML format,

- multi-pages form (form can be divided in different pages, displayed via the "next" button),

- form protection against spam, with turing image / spam filter (enter a random code displayed from image or text),

- customizable templates,

...
