I don't remember how, but with javascript you can disable form elements.  I just don't remember exactly how to disable them.

put disabled in the tag:

e.g

<input name="namehere" type="checkbox" disabled value="somevalue" />

You've "escaped" the closing tag of input which is XHTML so if you're actually intending to use XHTML you should give disabled a value of itself:

<input name="namehere" type="checkbox" disabled="disabled" value="somevalue" />
