[SOLVED] form will not submit data into table

hey guys... i have got this script that adds registered users into a table

for some reaosn every time the form is submitted it returns //Registration attempt failed so i belive there must be an error within the code

also just to add i have recently added a table to it called status... and set it to 0 ... does this need to be added into the script and is this why im getting the errors???
