Ajax form writing to MySQL

I'm looking for some help with creating a simple form that will display 2 input fields, pull existing data for the respective fields from MySQL database, allow write-over and submit. On Submit it will update the database and refresh the fields without page load.

Are you willing to pay us to do this job for you? if  not please search ajax tutorials on the net or the ajax start example on top of this section.  I believe its a very nice tutorial.
