I'd probably create a hidden div, which is made visible and repositioned next to the mouse cursor by writing a JavaScript function which is called when the user positions their mouse over the image. In the same function make an AJAX call that drops the larger image into the div.

 

Or a simpler method would just be to create a function that is called onmouseover that replaces the src of the image you are currently over for the larger version, then back to the smaller one onmouseout.
