I have Problem After send my form "Incorrect Table name"

Dear every body,

I'm very new in database and I'm wondering if somebody can help me.

I made a database with my sql and made a connection with Dreamweaver, Now I can see the result on web, but as sson as fill out the form "Insert record" I faced with "Incorrect Table Name" .

Please help me !!
