I'm trying to add records to a table which I can through phpmyadmin, but I'm making it where users can add records too.

 

Here's the code for the form:

<html>
<body>

<form action="insert.php" method="post">
Game <input type="text" name="game" />
Friend Code<input type="text" name="fc" />
Comments <input type="text" name="comments" />
<input type="submit" />
</form>

</body>
</html>

 

and the code for the insert.php:

  }
