How can I display my data like this?

I want to basically have each record have it's own table... The top line have a different color background and be First Name Last Name (which is field_1 field_2)

and then the rest of the table I want to have a white background and display

field_3

field_4

field_5

 

I just don't know how to display my data. That is what I am trying to learn...
