A "clear button" with Javascript "Are you sure you want to clear.."

I would like to code a clear button within my php code that will delete all fields in the table for the selected item

 

At present the button looks like this

Before the SQL takes place I would like a popup box asking the user if they are sure they want to clear ALL fields

 

I assume the SQL should be "Delete * FROM Product WHERE ProductNo='$name' "
