Please help! Making a PHP file... I am totally confused.....

This is the php  script which i made... but it is not working properly.... please help

 

the name of this file is

"Not working properly" gives us absolutely no clue on what the problem is, the error, what it doesn't do, what it should do...

i want this php file to send a feedback to my mail id with the requested data which i have mentioned in the php file next to  mail() function.....

 

 

Check your names.  You use different names for the same thing.  For example you have:

 

$EmailID=$_request['EmailID']; 

 

and

 


 
Change the input one to EmailID and it should work, but double check all of them.  Why would you have a bang in the name anyway?
 
Your method is also POST, so you should use POST, although the REQUEST should still work. 
 
And use [code]
tags please.
