Uploading multiple images in database with same value from one field in form

Dear all,

I have a problem that I need to be helped with using one form and storing the images in database. I would like to upload multiple images with same title from "title" field.

Can somebody help me with this?

you should not store images in a database. store the image as images, and store their names in a database.
