You would do the survey and check it's completion and everything in PHP. Javascript would just be used to make it look pretty and avoid page refreshes where you don't need them. You could overlay and darken the background like most people do, then just put the page into the div you centered on the page.

I'm sorry but do you even download a game at the end of that? Most of those things just loop through an endless amount of surveys, they're a scam. If you're wanting to create a scam we won't help.. Not that I'm trying to accuse you of scamming. Just asking.

I somehow doubt they're permitted to give out free downloads for Battlefield 3, especially considering they're using a Dutch domain from Blogspot...

Also had a quick look at it, and I can assure you all that the legal department of EA would have never signed off on that text. Too many of those legalese details missing.
