Looks good. I like it. You could make money with this IMO. My university here does be constantly handing out course review forms - and its a PITA reviewing and collating them.

1 suggestion -> when you look for your college - or subject even - and there isn't one - well instead of giving a blank list - it would be nice to say - "there are no subjects listed for this choice". etc. etc.

Overall not bad. If you are going to get schools/colleges using this -> I dont think the google ads would be a good idea. What you could have is a seperate sign on page for registered members - bypassing the ads. Get colleges/schools to pay for your service and remove the ads.

I've always wondered if there was an all around solution to this problem.  I can get them to line up in IE, but not Firefox, then with Firefox and not IE.  Is there a way to fix this or is this too complicated (meaning it's conditional)

i went to search for my college (which i realize won't be found since i'm in canada, but that's beside the point), but it didn't remember whether i was in a college or a high school when it loaded the state dropdown.  also doesn't remember after i've selected my state.  even though it doesn't affect functionality of the form, not repopulating the form makes people worry that it isn't working right.  plus, it shouldn't be hard to repopulate.

PS:  what's in your display?  i can't tell if that's a ketone near that methyl terminus or something else.
