On Windows, the php .zip package includes all the necessary files that php needs to use mysql. Short version - All you need to do is install and configure the Apache .msi package, add the php .zip package, install the mysql .msi package, and enable the mysql extension in php.

 

You could also just use one of the all in one packages - http://www.apachefriends.org/en/xampp.html
