Dropdown Is Not Populating From Sql Server Database

I am working on this script to populate a dropdown list from the SQL server express database but each time i run the script i get these errors. I am honestly really new with sqlserv syntax

When I run the script above , this is the error i get now and im just stuck here, i have exhausted all my ideas on how to resolve the SQL server error
