When Programming Do You Keep Protocol of Every Decision Made?

The title describes the question.

 

If you do so, how do you keep protocol of the decisions made? How would such protocol look like in a way that one could come back half a year later and still know exactly what has happened and which decisions have been made so that one can follow back the progress of the project.

 

I would appreciate tips.

Yup. Revision control system, emails, project management systems, etc. Just log everything

Suggestions for my case?

 

It is a project I am reckless with, it is one of the quick projects you mock up put up and want see what will happen with it. Quick decisive changes are pre-programmed.
